A. Attend the Main Conference
only:
A. Friday, November 7th at
7:00 pm – Sunday, November 9th at 6:30 pm.
B. Attend the Main Conference PLUS
pre/post conference workshops.
C. Attend pre/post conference workshops
only.
Registration: All fees
for the main conference and pre/post conference workshops
are
listed on the registration form. Complete the entire form.
Payment in full must accompany
your registration. Please note there are no refunds once you
have registered.

Registration Confirmation: You will receive a confirmation letter and your workshop
schedule approximately 3 weeks prior to the event. If you do not receive it, please contact

Special Needs: If you
have special needs, please indicate this in the appropriate
area on the registration form and a staff member will contact
you.
Certificate of Attendance:
There is a $15 pre-paid fee for Certificate of Attendance.
Lunch: A box lunch is
included in the Main Conference fee for Saturday & Sunday.
Dinner: A delicious buffet
dinner is available as an OPTIONAL selection for both Friday
and Saturday presented by the Sheraton. The buffet will include
meat, vegetarian and vegan selections for an additional $30
per meal per person. Pre-registration for the buffet is requested.
Parking: Parking is FREE
for conference participants!

Workshop Changes: All
changes to your workshop selections MUST be submitted in writing
or by calling our office no later than October 15, 2008. Changes
